Comprehensive Cancer Survivorship Clinic Development

The population of US cancer survivors is projected to exceed 20 million by 2026. The most common cancers survived by men include prostate cancer, colorectal cancer, and melanoma skin cancer. The most common cancers survived in female patients include breast cancer, uterine cancer, and colorectal cancer. Most cancer survivors are 65 years old or older and have been diagnosed within the last 10 years. These individuals carry unique challenges in ongoing care due to underlying health conditions that may have been exacerbated by cancer directed therapy. Healthcare systems across the US are shifting focus toward population health including ThedaCare Health System. The focus on population health is enriched by ThedaCare’s mission to empower individuals to live their unique best lives. To meet the changing focus of health care ThedaCare Cancer Center is committed to enhancing the delivery of care to the growing population of cancer survivors. By providing comprehensive summary of cancer directed therapies, highlighting the latent effects and surveillance needs to continue to live a healthy life a dedicated survivorship program is necessary. Through evaluation of current programs providing comprehensive care to cancer survivors, ThedaCare will open a dedicated survivorship clinic to meet the needs of this growing population. A comprehensive cancer survivorship clinic aligns with ThedaCare mission, vision, and values of empowering patients to live their unique best lives as our health system transitions into a population health focused care delivery model.
