Healthcare POA requirements (All 50 States)

For my capstone experience I interned at Froedtert Health in Milwaukee, WI.  My work for Froedtert Health was completely 100% remote and a completion of 80 hours of research.  My project consisted of researching all of the 50 states Healthcare Power of Attorney requirements.  The research consisted of what the signature requirements are for a Healthcare POA form and whether the document needed witnesses and or notarization.  I also needed to research if the form had a release of information statement for HIPAA guidelines.

I spent one day at Froedtert Health to do the orientation.  I was able to sit with Julie Albright and shadow her daily work and go over some policies and procedures.  I then connected with Melissa Ludin to talk about my project.  From there on I worked remotely and connected with Melissa online through email and Webex meetings.

I would like to thank Julie Albright, Melissa Ludin, and Betty Rockendorf for being able to setup my placement capstone at Froedtert Health Milwaukee on such short notice.  The experience was great and convenient by doing it remotely.  I very much enjoyed doing my project and hope to take my learning experiences with me on my future opportunities.
