Indexing of Various PHI Records According to Bellin’s Record Retention Guidelines

I had the great privilege of completing my capstone experience at Bellin Hospital, my current place of employment. I spent 84 total hours on site working on my capstone project. My project consisted of three separate smaller projects. The first project I completed was indexing deceased patient records and creating a log prior to destruction. This involved data entry within Excel as well as verification of all patient record information. The second project I worked on was indexing various records from the pulmonary clinic. This also involved creating a log prior to the destruction of these records. This involved verification of last date of service as well as destruction year. The final project I completed was indexing various historic cardiac records. For this part of the project, I interacted with Epic to locate whether these files had been scanned into the EHR. I again created a log for this information. For this particular project, I went through over 1400 patient records. My site supervisor, Kara, provided me with Bellin Health’s record retention guidelines which served as a reference throughout all three of my projects. I had to make sure all destruction dates followed these guidelines.

I also did complete some shadowing within the department as well. I shadowed the scanning role, documentation analysis role, physician suspension role, and ROI role.
