Western Wisconsin Health (WWH) is a rural health clinic (RHC) anticipating future payment model changes that could significantly impact the organization, and WWH wants to be proactive in identifying other sustainable payment models which are available. The potential of using population health and value-based care programs may allow for another income stream. Population health work requires upfront investment of time and resources to see the return on investment. The purpose of this project is to evaluate existing capabilities and future needs for a population health model for improving health outcomes at WWH.
