Quality Improvement Training Project

My capstone project consisted of working on updating our workflows for documenting our care gaps/quality measures for patients. Updating these will increase the outcomes of our quality measures to reach our goals. While it was determined we did not need to update any workflows, we did discuss that the staff were not updating the information properly in the chart. While they were updating Health Maintenance for the patient they were not giving notification to the HIM staff or printing out the document for scanning into the chart. The training material was to be a PowerPoint that consisted of the new workflows but since we did not need new workflows it was changed to a PowerPoint video of how to update Health Maintenance and review Care Everywhere to find the results for the patient. Once the video was completed it will to be sent to the staff for training.  There were some hiccups with the video but I was able to get that fixed and the video was completed. It will be updated each year with the new outcomes and sent for our annual Quality Improvement Training for staff. The next task was to review the reports that were initially ran and those patients that did not meet the measure I updated the Care Everywhere to find the results of the measures if the patient completed it. I was able to find a few patients that had results and uploaded those into the patient chart. When I reran the reports we were able to see positive changes in each of the measures. The updates to the measures were sent to my site supervisor and I was asked to present them to our medical director and the other staff in the Medical Ops meeting for this month.
